By submitting this application I confirm that:

1. All information provided in this application is true, complete and accurate to the best of my knowledge. I understand that providing false or misleading information may result in the withdrawal of any offer of membership.

I consent to ECAHLI and the ECAHLI Foundation storing and processing the personal and medical information provided in this form for the purpose of reviewing my application, assessing my care needs, and managing my membership. My information will not be shared with third parties without my consent except where required by law.

3. I understand that submission of this form does not constitute acceptance into ECAHLI and that all applications are subject to review, availability, and approval.

4. I understand that the information provided in Section 5 (Medical Overview) is strictly confidential and will be used only by ECAHLI's medical and care team to ensure appropriate accommodation and care provision.
